AcneLase: A Different Approach to Treating Acne

EXPERIENCE EVIDENCE-BACKED ACNE CLEARANCE

AcneLase is the dual-pulse Nd:YAG acne protocol developed by Dr. Jetty at Princeton Aesthetics, combining long-pulse and short-pulse 1064 nm energy in a single session to clear active inflammatory acne and refine post-acne skin without carbon suspension or concurrent oral antibiotics.

THE DUAL-PULSE ND:YAG PROTOCOL

LONG-PULSE ND:YAG

Sustained deep dermal heating that targets the sebaceous gland and reduces the inflammatory drive behind active acne. Comfortable in clinic, no anesthesia required.

SHORT-PULSE ND:YAG

Higher peak-power 1064 nm energy that adds photo-disruptive action against bacterial colonization and stimulates dermal remodeling for skin quality and early scar improvement.

DUAL-MECHANISM IN ONE SESSION

Long-pulse and short-pulse Nd:YAG are sequenced back-to-back in the same visit. The combination addresses active acne and post-acne skin in a single treatment block, not separate appointments.

What is AcneLase?

The Dual-Pulse Solution

AcneLase is the in-clinic protocol that combines long-pulse and short-pulse 1064 nm Nd:YAG energy in a single session to address the multiple drivers of acne at once. Each session targets sebaceous gland activity, post-inflammatory erythema, and the dermal architecture beneath active and resolving lesions. Most patients complete a series of six to eight sessions, spaced two to four weeks apart.

TREATMENT APPLICATION

Each session begins with long-pulse Nd:YAG delivered across the affected zones. Short-pulse Nd:YAG follows immediately in the same session to add photo-disruptive action and trigger collagen remodeling. Most face-only sessions take about thirty to forty-five minutes.

EFFECTIVE FOR ACTIVE ACNE AND POST-ACNE SKIN

AcneLase addresses inflammatory and cystic acne, post-inflammatory erythema, post-inflammatory hyperpigmentation, recurrent and antibiotic-resistant acne, and early to moderate atrophic scarring. The protocol is designed for patients who want clearance and skin quality improvement without long-term oral antibiotics or isotretinoin.

What Sets AcneLase Apart?

AN EVIDENCE-BACKED DUAL-PULSE APPROACH

AcneLase departs from the conventional acne paradigm of months of topicals, antibiotics, and isotretinoin. The protocol is a procedural, dual-pulse Nd:YAG treatment that addresses the underlying biology of acne in the dermis itself.

NO CARBON SUSPENSION REQUIRED

Earlier laser-acne protocols relied on a topical carbon lotion to enhance laser absorption. AcneLase produces measurable clearance without the carbon step, eliminating the messy preparation and the questions about long-term safety of repeated carbon exposure.

NO CONCURRENT ORAL ANTIBIOTICS

The protocol was validated with patients off all oral antibiotics and topical acne medications during therapy. Patients who want to stop the cycle of recurrent doxycycline or minocycline courses have a real procedural alternative.

SUSTAINED BENEFIT BEYOND TREATMENT

Published data demonstrate maintained improvement at a mean of twenty-two months after the active treatment phase ends. AcneLase is not a temporary surface treatment that fades when sessions stop.

TREATS ACTIVE ACNE AND POST-ACNE SKIN TOGETHER

Most acne protocols treat the acne now and the scars later. AcneLase addresses inflammation, post-inflammatory erythema, and early scar architecture in the same session, shortening the overall timeline to clear skin.

The dual-pulse Nd:YAG approach is the foundation of how acne can be treated procedurally. Dr. Jetty performs every AcneLase session personally and designs a session count tailored to disease severity and skin type.

How does AcneLase work?

Mechanism of Action

Each AcneLase session sequences two clinically distinct pulses in a deliberate order. Long-pulse Nd:YAG addresses the deep inflammatory and sebaceous components first; short-pulse Nd:YAG follows to add photo-disruption and dermal remodeling. The combination is what produces both clearance and skin quality improvement.

STEP 1: LONG-PULSE ND:YAG

Long-pulse 1064 nm energy is delivered across the affected zones. The sustained deep heating reduces sebaceous gland activity, calms the inflammatory milieu around active lesions, and warms the dermis in preparation for the second pulse.

STEP 2: SHORT-PULSE ND:YAG

Short-pulse 1064 nm energy follows immediately in the same session. The higher peak-power delivery adds photo-disruptive action against bacterial colonization and initiates collagen remodeling in the upper and mid dermis.

STEP 3: COOLING AND POST-CARE

Each session ends with cooling and post-care guidance. Sunscreen is non-negotiable, gentle skincare is preferred for forty-eight hours, and patients return for the next session in two to four weeks.

SEBACEOUS GLAND TARGETING

Reducing the root driver.
The long-pulse component delivers heat at the depth of the sebaceous unit. Reducing gland activity reduces the substrate for acne formation, not just the lesions already visible.

PHOTO-DISRUPTIVE BACTERIAL REDUCTION

The short-pulse contribution.
The short-pulse component adds a photoacoustic effect that contributes to bacterial reduction within the follicular unit and around active lesions.

POST-INFLAMMATORY ERYTHEMA CLEARANCE

Treating the red marks acne leaves behind.
1064 nm Nd:YAG has a real vascular signature. The combined pulses fade the persistent redness that follows resolved acne lesions over the treatment series.

EARLY SCAR REMODELING

Skin quality alongside clearance.
Short-pulse Nd:YAG initiates collagen remodeling in the upper dermis. Patients with early atrophic scarring see softening of the architecture across the treatment series.

EVIDENCE-BACKED AT EVERY STEP

Validated dual-pulse protocol.
The combined long-pulse and short-pulse Nd:YAG approach was validated by Bakus et al. in 2018 with sustained benefit at twenty-two months. AcneLase carries that evidence into clinic.

The two pulses are not interchangeable. Long-pulse alone treats the inflammation; short-pulse alone treats the skin quality. Sequencing both in one session is what makes AcneLase a different procedural treatment for acne.

Who Is a Candidate for AcneLase?

AcneLase is appropriate for teens and adults with active inflammatory acne, post-inflammatory erythema, recurrent or antibiotic-resistant acne, and early to moderate atrophic scarring. For patients with predominantly pigmentary sequelae, we also offer PicoGenesis, and for predominantly comedonal acne or routine skin clarification we offer HydraFacial as complementary protocols.

ACTIVE INFLAMMATORY ACNE

Papulopustular and nodulocystic disease.
Patients with active inflammatory and cystic acne who want a procedural treatment that addresses the disease at the dermal level.

ANTIBIOTIC-RESISTANT OR RECURRENT ACNE

When the same prescription cycle keeps repeating.
Patients who clear on oral antibiotics and relapse when the course ends. AcneLase offers a different point of attack on the disease.

PATIENTS DECLINING ISOTRETINOIN

A procedural alternative.
Patients who are candidates for isotretinoin but prefer a non-systemic option. AcneLase is not a substitute for isotretinoin in every case, and Dr. Jetty discusses both pathways during consultation.

AcneLase is not for patients with active herpes simplex outbreaks, patients on photosensitizing medications, or patients with very recent isotretinoin use. Dr. Jetty reviews every candidate personally and recommends the appropriate timeline for treatment.

Frequently Asked Questions

Is AcneLase safe for all skin types?

Yes. 1064 nm Nd:YAG has minimal absorption by epidermal melanin, which makes it the wavelength of choice for acne treatment across the full Fitzpatrick range, including darker skin types. Dr. Jetty calibrates fluence and pass density to skin type at every session. Patients on photosensitizing medications and patients with very recent isotretinoin use need timing adjustments, which are reviewed during consultation.
Most patients complete a course of six to eight AcneLase sessions, spaced two to four weeks apart. The published dual-pulse Nd:YAG data on which AcneLase is based reported strong clearance with a minimum of eight sessions and durable benefit at twenty-two months after the active treatment phase. Patients with more severe disease or with scarring may benefit from additional sessions, and maintenance treatments every two to three months are recommended once clearance is achieved.
The treatment is performed in-clinic with no anesthesia required. The long-pulse component feels like sustained warmth across the affected zones. The short-pulse component feels like a series of light, fast snaps. Most face-only sessions take thirty to forty-five minutes from start to finish. Patients return to normal activity immediately after.
Downtime is minimal. Mild transient redness and warmth are typical for the first few hours after treatment. Most patients return to work, school, and social activity the same day. Makeup can be applied the next morning. The procedure does not require time off.
The published safety profile of dual-pulse Nd:YAG for acne is excellent. The most common finding is brief transient erythema. Rare findings include small areas of post-treatment darkening that resolve and short-lived sensitivity in the treated zones. Patients with darker skin types are managed with conservative settings to keep post-inflammatory pigment risk low.
Sunscreen is required after every session, ideally a mineral SPF 30 or higher. Gentle cleansers and moisturizers are preferred for the first forty-eight hours. Topical acne medications that were discontinued before the series may or may not resume during the treatment block, depending on Dr. Jetty’s recommendation for each patient.
Oral antibiotics work systemically and clear acne while in use, but relapse is common when the course ends. Isotretinoin remains the most effective systemic option for severe disease but requires monitoring, has side effects, and is contraindicated in pregnancy. AcneLase is a procedural treatment that addresses the dermal biology of acne without systemic medications, supported by published evidence of sustained benefit beyond the treatment phase. Compared to older laser-acne protocols that required a topical carbon suspension, AcneLase produces measurable clearance without the carbon step. We discuss the trade-offs of each pathway during consultation.
