Emsella for Nocturia

NON-INVASIVE PELVIC FLOOR THERAPY FOR NIGHTTIME URINATION

Emsella is a noninvasive pelvic floor therapy offered by Dr. Jetty at Princeton Aesthetics that can reduce nighttime awakenings to urinate when the underlying cause is an overactive bladder or a weakened pelvic floor. Every consultation begins with a physician workup to identify the true cause of your nocturia, because Emsella is not the right answer for every case.

Nocturia Isn't One Problem. It's Three. Treatment Depends on Which One.

EVIDENCE-BASED THREE-CAUSE PROTOCOL

Nocturia has three distinct mechanisms, and treatment depends entirely on which one is driving your symptoms. Emsella helps with one of them, does not help with the others, and identifying which category applies to you is why every consultation begins with a proper physician workup. Nocturia is not one condition. It is a symptom with three very different root causes.

THE PHYSICIAN-WORKUP DIFFERENCE

OVERACTIVE BLADDER + PELVIC FLOOR WEAKNESS

When the bladder contracts too easily or the pelvic floor no longer supports it well, the sensation of urgency wakes you at night even when the bladder is not full. This is the category where Emsella is genuinely helpful.

NOCTURNAL POLYURIA

Overnight overproduction of urine from age-related ADH decline, heart conditions, sleep apnea, uncontrolled diabetes, evening fluid intake, or medications. Emsella will not help. We identify and treat the underlying cause directly.

SLEEP DISORDERS, ESPECIALLY SLEEP APNEA

Sleep apnea is one of the most common hidden causes of nocturia and is frequently missed. When apnea is treated, nocturia often resolves without any bladder-directed treatment. We screen and refer appropriately.

What is Emsella for Nocturia?

The Pelvic-Floor Chair for Sleep Disruption

Emsella is a non-invasive pelvic floor therapy offered by Dr. Jetty at Princeton Aesthetics using the BTL Emsella chair. Each 28-minute session uses HIFEM (High-Intensity Focused Electromagnetic) technology to trigger thousands of supramaximal pelvic floor contractions, stronger and more numerous than anything achievable through voluntary exercise or standard Kegel training. When nocturia is driven by pelvic floor weakness or an overactive bladder, this reconditioning translates directly into improved bladder storage capacity and fewer nighttime awakenings for appropriately selected patients.

TREATMENT APPLICATION

Patients remain fully clothed and seated on the Emsella chair for 28 minutes. No needles, no anesthesia, no lidocaine, no downtime. Most patients read, work, or use their phone during the session and drive themselves home immediately after.

EFFECTIVE FOR NOCTURIA AND URINARY URGENCY

Emsella addresses nocturia caused by pelvic floor weakness or overactive bladder in both men and women. Post-prostatectomy men and post-menopausal women both benefit equally. Emsella is gender-neutral and is not appropriate for nocturia driven by nocturnal polyuria, sleep apnea, or other non-pelvic causes (identified through the physician workup at consultation).

What Sets Emsella for Nocturia Apart?

AN EVIDENCE-BASED PELVIC-FLOOR PROTOCOL

PHYSICIAN-DIRECTED WORKUP

Every nocturia consultation is done by Dr. Jetty personally. Nocturia is a multifactorial symptom, not a diagnosis, and the workup requires physician-level judgment.

HONEST CANDIDACY ASSESSMENT

We tell you if Emsella is not the right answer and refer you to the appropriate specialist. Nocturia driven by sleep apnea, kidney, or cardiac causes deserves the right treatment.

NON-INVASIVE, NO DOWNTIME

No needles, no anesthesia, no recovery period. Drive home and return to work immediately after each session.

GENDER-NEUTRAL CARE

Emsella is equally effective for men and women. Post-prostatectomy men and post-menopausal women both benefit.

STANDARD-OF-CARE EVALUATION

Voiding diary, urinalysis, post-void residual, sleep apnea screen, medication review. Not a sales visit.

ONGOING PHYSICIAN OVERSIGHT

Every session is under physician supervision. If your response is not what we expect, we adjust or refer.

How does Emsella work for nocturia?

Mechanism of Action

Each 28-minute Emsella session delivers HIFEM energy that penetrates the pelvic floor musculature and triggers thousands of supramaximal contractions no voluntary effort could achieve. Over a course of treatment, this reconditions the muscles that support bladder capacity, urethral closure, and nighttime bladder stability. When nocturia is driven by pelvic floor or bladder mechanisms, this translates into fewer awakenings and longer stretches of sleep.

STEP 1: PATIENT POSITIONING

You sit fully clothed on the Emsella chair. Legs relaxed, pelvis centered over the HIFEM applicator built into the seat. No preparation, no undressing, no injection.

STEP 2: HIFEM ACTIVATION

The device generates a focused electromagnetic field that penetrates the pelvic floor musculature, triggering supramaximal contractions of the pubococcygeus, iliococcygeus, and puborectalis muscles.

STEP 3: MUSCLE RECONDITIONING

Over 28 minutes, the pelvic floor performs the equivalent of thousands of Kegel-style contractions, well beyond any voluntary session. Muscle fiber recruitment, endurance, and coordination all improve.

STEP 4: POST-SESSION RETURN TO ACTIVITY

No recovery period. You stand up, walk out, and drive home. Return to work, exercise, or normal activity immediately.

MUSCLE FIBER RECRUITMENT

HIFEM recruits both slow-twitch and fast-twitch pelvic floor fibers, which voluntary Kegels rarely activate together. Full-spectrum reconditioning is why the results outperform home exercise alone.

BLADDER STORAGE CAPACITY

Stronger pelvic floor support translates directly into improved bladder storage before the urge-to-void signal fires. That is the mechanism that reduces nighttime awakenings in urgency-driven nocturia.

URETHRAL CLOSURE STRENGTH

Pelvic floor strengthening improves urethral closure pressure at rest, meaning less involuntary leakage and less urgency-triggered nighttime wake-up in patients with mixed incontinence.

NEUROMUSCULAR RETRAINING

Repeated supramaximal contractions retrain the neuromuscular pathways controlling bladder and pelvic floor coordination. This is why the benefit persists beyond the treatment course, with maintenance sessions preserving it.

SYNERGY WITH OTHER PELVIC HEALTH SUPPORT

Emsella works alongside behavioral changes (fluid timing, evening caffeine reduction), pelvic floor physical therapy, and appropriate medical management of any concurrent conditions identified in your workup.

Who Is a Candidate for Emsella Nocturia Treatment?

Emsella is a strong candidate treatment for nocturia when your physician workup identifies pelvic floor weakness or an overactive bladder as the underlying driver. It is not appropriate for every case, and part of the value of a physician consultation is knowing when to recommend it and when to refer you to a specialist for a different cause. The following patient categories are typical candidates.

OVERACTIVE BLADDER WITH NOCTURIA

Adults whose nocturia is driven by urgency episodes from an overactive bladder, particularly when daytime urgency is also present.

POST-CHILDBIRTH PELVIC FLOOR WEAKNESS

Women whose nocturia developed or worsened after vaginal delivery, especially with a history of large babies, multiple deliveries, or prolonged pushing.

POST-MENOPAUSAL PELVIC FLOOR CHANGES

Post-menopausal women with age-related and hormonal pelvic floor deconditioning presenting as nighttime urgency and incomplete storage.

POST-PROSTATECTOMY MEN AND AGE-RELATED CHANGES

Men following prostate surgery, and men with age-related pelvic floor muscle loss contributing to nighttime urinary changes. Emsella is fully gender-neutral.

Frequently Asked Questions

Is Emsella FDA-approved for nocturia?

Emsella is FDA-cleared for stress urinary incontinence, urge urinary incontinence, and mixed incontinence in men and women. Use for nocturia specifically is off-label and appropriate when nocturia is driven by pelvic floor or bladder mechanisms rather than other causes. We discuss this openly during your consultation.

Emsella can significantly reduce nocturia episodes in appropriately selected patients, typically by 30 to 60 percent of baseline episodes. It is not a cure for all nocturia, and the response depends on identifying the correct underlying mechanism. Patients whose nocturia is caused by nocturnal polyuria or sleep apnea will not benefit from Emsella regardless of how many sessions they complete.

Most patients notice improvement within the initial 6-session course over 3 weeks. Continued improvement often occurs over the following 4 to 8 weeks as the pelvic floor muscles remodel. Maintenance sessions every 3 to 6 months preserve the effect.

Emsella is not painful. Patients feel intense pelvic muscle contractions, which can feel unusual but is not painful. Most patients read, work, or use their phone during the 28-minute session.

No. Metal implants in the treatment field are a hard contraindication for Emsella regardless of the metal’s MRI-conditional rating. Emsella uses pulsed electromagnetic energy, which behaves differently from static MRI magnetic fields and can induce currents or heating in metal implants. If you have a UroLift, prostate implant, copper IUD, sacral neuromodulator, or similar pelvic implant, we will discuss alternative options at your consultation.

No referral is needed for the consultation. If our workup identifies a cause of nocturia outside our scope (sleep apnea, cardiac, kidney, BPH obstruction), we coordinate with your primary care physician or refer to the appropriate specialist.

Pricing is discussed at your consultation once candidacy and session count are confirmed. Emsella for nocturia is not typically covered by health insurance. Package pricing is available for the initial 6-session course.

Evaluated and Planned by Dr. Jetty

Every nocturia consultation and every treatment plan is authored personally by Dr. Jetty. No cookie-cutter protocols, no non-physician recommendations.

Standard-of-Care Workup

Voiding diary, urinalysis, post-void residual, sleep apnea screen, medication review. The evaluation is standard-of-care internal medicine before any pelvic floor recommendation is made.

Honest Referral When Needed

If your nocturia is caused by sleep apnea, cardiac, kidney, or BPH obstruction, we refer to the right specialist rather than recommend Emsella. Your outcome matters more than our service revenue.

Gender-Neutral Care

Emsella is equally effective for men and women. Post-prostatectomy men and post-menopausal women both receive appropriate physician-directed treatment here.

Non-Invasive, No Downtime

No needles, no anesthesia, no recovery period. Drive yourself home, return to work, resume exercise immediately after each session.

Personalized Session Cadence

Standard initial course is 6 sessions twice weekly, then maintenance every 3 to 6 months. Your specific cadence is customized based on response and your individual pattern.

Same-Day Consultation Availability

We schedule consultations promptly. If you are dealing with sleep disruption from nocturia, waiting weeks to be seen makes no sense.

Comprehensive Aftercare Guidance

Beyond the sessions themselves, we counsel you on fluid timing, caffeine and alcohol adjustments, and daytime voiding intervals that support and extend the treatment benefit.

Continuity of Care

The same physician who evaluates you initially oversees your treatment course and any maintenance sessions. Continuous physician relationship, not a rotating cast.
