Emsella helps with one of them, does not help with the others, and identifying which category applies to you is why every consultation begins with a proper physician workup. Emsella is a strong candidate treatment for nocturia when your physician workup identifies pelvic floor weakness or an overactive bladder as the underlying driver. It is not appropriate for every case, and part of the value of a physician consultation is knowing when to recommend it and when to refer you to a specialist for a different cause. The following patient categories are typical candidates.
Adults whose nocturia is driven by urgency episodes from an overactive bladder, particularly when daytime urgency is also present.
Women whose nocturia developed or worsened after vaginal delivery, especially with a history of large babies, multiple deliveries, or prolonged pushing.
Post-menopausal women with age-related and hormonal pelvic floor deconditioning presenting as nighttime urgency and incomplete storage.
Men following prostate surgery, and men with age-related pelvic floor muscle loss contributing to nighttime urinary changes. Emsella is fully gender-neutral.
Emsella is FDA-cleared for stress urinary incontinence, urge urinary incontinence, and mixed incontinence in men and women. Use for nocturia specifically is off-label and appropriate when nocturia is driven by pelvic floor or bladder mechanisms rather than other causes. We discuss this openly during your consultation.
Emsella can significantly reduce nocturia episodes in appropriately selected patients, typically by 30 to 60 percent of baseline episodes. It is not a cure for all nocturia, and the response depends on identifying the correct underlying mechanism. Patients whose nocturia is caused by nocturnal polyuria or sleep apnea will not benefit from Emsella regardless of how many sessions they complete.
Most patients notice improvement within the initial 6-session course over 3 weeks. Continued improvement often occurs over the following 4 to 8 weeks as the pelvic floor muscles remodel. Maintenance sessions every 3 to 6 months preserve the effect.
Emsella is not painful. Patients feel intense pelvic muscle contractions, which can feel unusual but is not painful. Most patients read, work, or use their phone during the 28-minute session.
No. Metal implants in the treatment field are a hard contraindication for Emsella regardless of the metal’s MRI-conditional rating. Emsella uses pulsed electromagnetic energy, which behaves differently from static MRI magnetic fields and can induce currents or heating in metal implants. If you have a UroLift, prostate implant, copper IUD, sacral neuromodulator, or similar pelvic implant, we will discuss alternative options at your consultation.
No referral is needed for the consultation. If our workup identifies a cause of nocturia outside our scope (sleep apnea, cardiac, kidney, BPH obstruction), we coordinate with your primary care physician or refer to the appropriate specialist.
